What you'll do

• Review plan participants’ clinical eligibility for home infusion therapy
• call patients to discuss home infusion benefits and address questions
• educate patients and caregivers about the home infusion process
• schedule initial appointments with in-network providers
• facilitate transitions from current care settings to home
• verify clinical documentation, physician orders, and payer authorizations
• coordinate readiness, delivery logistics, and nursing schedules with providers
• monitor patients after transition and escalate concerns
• identify and help resolve barriers
• conduct assessments, develop care plans, evaluate effectiveness, and document activities.
