What you'll do

• Build relationships with hospitals, care facilities, physicians, senior organizations, and community partners
• represent CareDiem at community events
• follow up with prospective clients and referral partners
• maintain the lead pipeline
• meet with prospective clients and families
• conduct client assessments in homes or virtually
• coordinate start of care and documentation
• communicate client needs to the care team
• conduct follow-up assessments and family check-ins
• contribute ideas to company culture and caregiver support.
