Social Worker Assistant

The Social Worker Assistant role is onsite in Sodus, NY. Pay is $22–$24 per hour. The position is in the Social Services department at Sodus Rehabilitation and Nursing Center. The role supports residents and families through social services planning, assessments, therapeutic interventions, and coordination with facility teams and community partners. It contributes to resident well-being and continuity of care by supporting admissions, discharges, care planning, documentation, and quality and compliance activities.

What you'll do

• Assist with planning social services for residents, representatives, and families
• coordinate admissions and discharges
• participate in interdisciplinary care
• provide therapeutic interventions related to illness, impairment, addiction, disability, and separation from family or community
• complete and maintain resident assessments and medical-record documentation
• support social services policies and regulatory compliance
• participate in admission decisions and discharge planning
• coordinate with facility departments, medical staff, families, and community agencies
• support staff education and quality-assurance activities
• respond to resident and family concerns and monitor complaints and incidents.

Why this role

The posting requires at least two years of social work experience and at least one year of supervised experience in a healthcare facility’s Social Services department. A bachelor’s degree in social work or a related human services field is preferred.
