The Direct Support Professional – ICF Residential, 2nd Shift is an onsite, full-time role in Roxboro, NC, with starting pay of $14–$14.50 per hour. The set schedule is Monday through Wednesday, 3:00 p.m.–11:00 p.m., and every other Saturday and Sunday, 9:00 a.m.–9:00 p.m., for 36 scheduled hours per workweek. PCGH supports adults with intellectual and developmental disabilities and traumatic brain injuries, helping them live in a home with individualized care. The role provides hands-on personal and daily-living support to residents with significant medical and personal-care needs. By following individualized plans, monitoring changes, documenting care, and coordinating with nursing and clinical staff, the Direct Support Professional helps maintain residents’ safety, comfort, and continuity of care.

What you'll do

• Provide personal care, hygiene assistance, and support with daily routines
• assist with mobility, positioning, and transfers
• administer medications and treatments after required training and certification
• follow physician orders, care instructions, and Individual Service Plans
• monitor and promptly communicate changes in health, behavior, appetite, mobility, or condition
• support meals, activities, and appointments
• complete documentation
• coordinate handoffs with incoming staff and clinical team members.
