What you'll do

• Collaborate with the HCBS Supervisor and Programmer on individualized goals
• provide direct support that teaches skills, promotes independence, and encourages recreational and community participation
• use approved behavioral strategies and respect rights restrictions in service plans
• complete accurate daily documentation in Setworks
• assist with daily living, transportation, mobility, and safety
• administer and document medications according to the six rights
• communicate concerns and updates
• complete required training.
