The nurse coordinates case management, admissions, recertifications, assessments, and care transitions while providing clinical supervision and occasional skilled nursing visits. The role supports continued medical necessity, compliant documentation, communication across care teams, and quality outcomes for clients.

What you'll do

• Conduct client assessments
• manage starts of care, admissions, recertifications, transitional care, resumptions, clinical reviews, and other assessments
• supervise nursing and aide staff
• document and communicate patient status changes
• coordinate care
• provide occasional skilled nursing visits
• support orientations, in-service training, and supervisory reviews.
