The Registered Dietitian role is onsite in San Leandro, CA, at a long-term care facility. Pay is $35–$45 per hour. Healthcare Services Group supports clinical nutritional services and resident care. The dietitian assesses residents’ clinical nutrition needs, develops individualized care plans, and provides education and counseling. Collaboration with interdisciplinary teams helps support residents’ health and well-being while meeting applicable care regulations.

What you'll do

• Assess residents’ and patients’ clinical nutrition needs
• develop and document plans in the electronic medical record
• implement and evaluate individualized care plans using clinical findings, lab results, weight trends, intake, and comorbidities
• provide nutrition education and counseling
• collaborate with interdisciplinary teams
• follow CMS, state, federal, and company standards.
