What you'll do

• Provide restorative care, including exercises, transfers, and ambulation
• assist with bathing, dressing, and grooming
• monitor and report changes in resident condition
• develop and implement personalized care plans with the interdisciplinary team
• document progress and communicate with families and healthcare providers.
