What you'll do
- Coordinate preliminary and comprehensive Minimum Data Set (MDS) assessments, including CAAs, triggers, and plans of care, under RN supervision when the incumbent is an LVN.
- Ensure assessments are completed and signed on schedule, interview residents, help develop and update care plans, and evaluate resident conditions and medical information for needed assessment changes.
