Case Manager – Strive Recovery

Job overview

The Case Manager – Strive Recovery role is onsite in Waterloo, IA. Compensation is $26 per hour. The position supports Elevate CCBHC's Strive Recovery program for people enrolled in substance use treatment. The case manager coordinates recovery planning and services with clients, families, treatment providers, and community resources. The work supports transitions to housing and community settings, continued recovery planning, and follow-up after discharge.

What you'll do

• Develop recovery plans in coordination with counselors' treatment plans
• coordinate services and transitions to home communities, primary care, and other resources
• support individuals in Strive Dorms and coordinate behavior plans with resident assistants
• review Oxford-style housing applicants with the team
• document services
• help clients access community resources
• coordinate with HHS, probation officers, and other providers as appropriate
• address insurance coverage issues with clients and the utilization review team
• monitor referrals and assist the program director with reviews
• conduct post-discharge follow-up care.

Why this role
