What you'll do

• Utilizes the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care to patients.
• Facilitates health care services for patients in outpatient settings in cooperation with the health care team.
• Coordinates care, patient assessment, patient education, and various other nursing interventions.
• Collaborates with other inter-professional colleagues, including physicians, to plan, implement and evaluate outpatient care.
