Social Services Director

What you'll do

• Build working relationships with direct caregivers to support resident psychosocial well-being
• help identify and address problem areas
• maintain confidentiality and documentation
• coordinate referrals, including payer-source identification and review of patient information
• collaborate with patients or representatives, physicians, residents, nursing, rehabilitation, and social-work staff to support transfers
• communicate relevant information to physicians, supervisors, administrators, and other personnel
• perform other assigned duties.
