What you'll do

• Teach and assist clients with skills to increase independence and functioning
• use evidence-based practices in community and home settings
• participate in treatment meetings and assessments
• document interventions
• write individualized treatment plans and annual renewals
• help clients access services and providers
• collect information for assessments
• coordinate referrals and follow up
• develop case management service plans with members and treatment teams
• comply with insurance carrier requirements.
