Case Manager 1

The Case Manager 1 role is onsite in Lynchburg, VA, with a Monday–Friday schedule of 8:30 a.m.–5:00 p.m. Compensation is $18.37 per hour. The position supports individuals and families served by Horizon Behavioral Health. Under the supervision of a qualified case management supervisor, the case manager assesses needs, coordinates referrals and services, and follows up on supports. The role helps ensure clients receive appropriate services while maintaining effective relationships and accurate documentation.

What you'll do

• Provide case management to individuals and families
• establish linkages and referrals
• coordinate care and follow up on needed services and supports
• identify and assess client needs
• evaluate and help ensure necessary services are provided
• document in an electronic medical record
• make home visits and travel as needed
• transport clients or provide services at other Horizon locations when called upon.
