What you'll do

• Act as a liaison between patients, professional staff, and physicians
• provide care management and coordination for adults with complex conditions and social or educational needs
• collaborate with primary care providers, specialists, patients, and families
• assess, plan, implement, coordinate, monitor, and evaluate care options and services
• support quality improvement initiatives and adherence to safety standards.

Why this role

Requires an active Michigan registered nurse license. Experience in complex chronic disease management, a Bachelor of Science in Nursing, and five years of registered nursing experience are preferred. Schedule is Monday–Thursday in Harbor Springs and Fridays in Charlevoix, 8:00 a.m.–4:30 p.m.
