Nurse Case Manager

The Nurse Case Manager role is an onsite position in Freetown, MA. Compensation is $35–$38 per hour. The role is part of Crystal Springs’ Adult Nursing department, supporting adults with developmental disabilities in pursuing health, well-being, and greater independence. The nurse coordinates care for an assigned caseload, follows medical orders and care plans, and provides oversight and nursing support to individuals and staff. The role helps maintain continuity of medical care through assessments, documentation, appointment coordination, staff training, and communication with families and care teams.

What you'll do

• Oversee assigned homes and individuals, including in-home audits, medication and MAR reviews, and staff training
• complete monthly summaries and skin assessments
• conduct physical assessments and provide basic wound care as needed
• communicate with care teams, guardians, providers, and vendors
• maintain timely documentation
• coordinate appointments and follow-up care
• review tracking logs and suggest care interventions
• attend appointments and individual service plans as needed
• provide G and J tube care and staff training as needed
• maintain licensure and certifications.
