This position supports the rehabilitation department at Mantey Heights Rehabilitation & Care Center, a skilled nursing facility dedicated to resident-centered care. The role exists to evaluate residents, develop individualized treatment plans, and provide skilled therapy services to improve strength, mobility, balance, safety, and overall functional independence.

What you'll do

• Perform comprehensive physical therapy evaluations.
• Develop individualized treatment plans based on resident needs, goals, and physician orders.
• Provide skilled therapeutic interventions to improve mobility, strength, balance, endurance, and functional independence.
• Assess and recommend assistive devices, mobility equipment, and safety modifications.
• Monitor resident progress and adjust treatment plans as appropriate.
• Educate residents, families, and caregivers regarding treatment plans and discharge recommendations.
• Maintain accurate and timely documentation in accordance with regulatory and professional standards.
• Participate in interdisciplinary care planning meetings.
• Ensure compliance with all federal, state, facility, and professional practice requirements.
