What you'll do

• Complete Start of Care admissions throughout the Twin Cities Metro
• perform nursing assessments and OASIS documentation
• develop individualized care plans
• educate patients and caregivers
• coordinate with physicians, therapists, case managers, and the interdisciplinary team
• and support safe transitions from hospital to home.
