What you'll do

• Maintain caseloads and regular client contact in the community and office
• monitor and assess progress
• meet with clients individually or in groups
• collaborate with clients, families, case managers, and clinicians
• provide services that may include crisis intervention, advocacy, assistance, referrals, and transportation
• prepare clinical reports and individualized service plans
• maintain compliant, current records
• conduct family outreach and participate in the Loved Ones Group
• communicate with probation departments about client progress.
