What you'll do

• Assess and monitor residents’ nutritional status
• develop individualized care plans
• address weight changes, skin integrity, nutrition support, and special dietary needs
• monitor meals and prescribed diets
• collaborate with nursing, medical staff, speech-language pathologists, and other team members
• provide nutrition education
• review menus and diet changes for compliance
• oversee nutrition audits and weight monitoring
• document in the EMR and dietary/menu software.

Why this role
