What you'll do

• Conduct comprehensive nursing assessments and collaborate on individualized plans of care.
• Maintain accurate documentation, communicate with the care team, provide education and emotional support to patients and caregivers, and participate in admissions, care transitions, and on-call rotations.
• In an on-call capacity, respond to triage calls, perform visits and assessments, and coordinate care with the Administrator On Call.
