LB Homes provides long-term care services to residents. The coordinator oversees the Short Stay Unit while performing registered nurse functions, supporting resident care and coordinating staff, assessments, care planning, and unit operations in collaboration with the Director of Nursing.
What you'll do
- Oversee the Short Stay Unit
- perform registered nurse functions as assigned by the Director of Nursing
- coordinate care and services based on resident needs and assessments
- complete pre-admission and scheduled clinical assessments and care plans
- provide clinical leadership
- supervise, evaluate, and document staff performance with the Director of Nursing
- support onboarding and training
- maintain electronic health records
- promote safety and infection control
- respond to emergencies
- meet vulnerable adult reporting requirements.
