What you'll do

• Develop and maintain referral relationships with hospitals, physicians, assisted living communities, and skilled nursing facilities
• grow referral sources in the Fort Worth service area
• educate providers and community partners about hospice
• meet with patients and families to explain services and obtain consents
• coordinate with the clinical team on admissions
• represent the hospice as a community resource.
