Registered Nurse (RN) Case Manager, Home Care

The Registered Nurse (RN) Case Manager, Home Care role is primarily remote, with in-person patient visits as needed, for candidates based in Naples, Lehigh Acres, or Fort Myers, FL, and a caseload across Southwest Florida. Compensation is $38–$40 per hour. The role supports Family Pillars Homecare’s home-care services. The nurse manages and coordinates patient cases, updates individualized care plans, monitors patient needs, and coordinates authorized services with patients, families, providers, payers, and caregivers. The role helps maintain responsive, organized continuity of care while addressing service gaps and supporting quality documentation.

What you'll do

• Manage managed-care, workers’ compensation, and private-pay patient caseloads throughout Southwest Florida
• review and update individualized care plans with patients, families, caregivers, providers, and care-team members
• conduct in-home assessments and visits as needed
• monitor patient condition and coordinate follow-up
• track authorized services and requests for additional care
• communicate with payers, adjusters, case managers, and treating providers
• maintain clinical documentation and case updates
• collaborate with scheduling, HR, and clinical leadership
• assist with field-staff onboarding and orientation
• identify barriers to care and service gaps
• assist with daily quality assurance of notes and files.
