What you'll do

• Develop referral relationships with hospitals, assisted living communities, skilled nursing and rehabilitation facilities, physicians, home health agencies, hospice providers, and other referral sources
• make face-to-face visits and prospect for new relationships
• explain HomeWell services
• seek referrals and business
• maintain follow-up and relationship management
• track activities, opportunities, referrals, and results
• manage territory performance.
