What you'll do

• Provide in-person residential program coverage
• facilitate rehabilitation services and skills training
• monitor medication
• assist consumers with individual service plans
• facilitate group and individual activities
• transport consumers in company vehicles to access community resources
• document services in the electronic health record and log book
• support independent living skills
• communicate across shifts
• engage in crisis management.
