What you'll do

• Assess physical health, behavioral health, social service, and long-term support needs
• identify members at risk or who may benefit from alternative care or waiver programs
• obtain clinical data as directed by the responsible RN or care coordinator
• coordinate care and collaborate on person-centered service plans
• participate in team rounds
• travel to member homes and other locations for assessments and support.
