What you'll do

• Review and verify home infusion medication orders
• assess dose, route, frequency, duration, organ function, allergies, and interactions
• monitor pharmacokinetic dosing, particularly vancomycin and aminoglycosides
• review laboratory results and recommend therapy changes
• collaborate with healthcare professionals
• provide patient and caregiver education when appropriate
• oversee sterile preparations
• maintain documentation
• communicate medication and monitoring concerns
• assist with after-hours or urgent needs as part of the PRN team.

Why this role
