Registered Nurse Team Leader (Triage) — Hospice, Every Other Weekend

What you'll do

• Provide skilled nursing intervention and telephone support
• assess patient and family needs and determine whether a home visit is needed
• obtain orders and notify appropriate nurses
• initiate referrals and supplemental orders
• communicate patient concerns and orders to departments
• coordinate clinical services and weekend schedules
• review lab results and report critical values
• support staff and families during patient crises or death
• document and communicate activity summaries.
