What you'll do

• Coordinate nursing case management and direct nursing services
• assess participant needs and develop and monitor care plans
• review health-risk screening and AIMS results
• coordinate medication, treatment, appointments, and provider communication
• participate in interdisciplinary planning and hospital or program transitions
• document services and support regulatory compliance
• train staff on medication administration, proxy caregiving, healthcare plans, and clinical topics
• provide onboarding follow-up and competency support
• travel to program sites and external meetings
• rotate weekend on-call duties.
