Registered Nurse (RN) — Visiting Home Health

The Registered Nurse (RN) — Visiting Home Health role is an onsite, per-diem position based in Palm Desert, CA, with work performed on the road at patients’ homes. Pay is $50–$140 per visit. Alliance Health Services provides home health care and reports a five-star Medicare rating for care in Southern California. The nurse provides home visits and clinical care that help patients maintain independence. The role assesses patients, administers treatments, coordinates and updates care plans with the clinical team and physicians, and completes required documentation and supervisory visits.

What you'll do

• Provide home health visits
• perform comprehensive assessments and document findings
• obtain and document physician orders
• initiate and regularly reevaluate care plans in consultation with the Clinical Team Manager
• coordinate care and report significant changes
• complete recertifications, resumptions of care, transfers, and discharges as requested
• assess need for other team services
• perform Home Health Aide supervisory visits
• follow regulatory requirements.
