Community Success Coordinator

What you'll do

• Serve as primary contact for assigned senior living and long-term care communities
• build relationships with community leaders and staff
• conduct onsite visits
• address concerns and coordinate solutions
• support retention and resident participation
• identify opportunities to expand access through referrals and engagement
• lead kickoff meetings
• coordinate with operations, scheduling, and clinical teams
• maintain relationship records and engagement metrics
• travel 60–75% of the time, including occasional overnight trips.
