Community Health Coordinator

Job overview

The Community Health Coordinator role is onsite in Olean, NY, with screening and referral work conducted by phone, virtually, and in person, including off-site community programs. Compensation is $20–$24 per hour. The role supports the YMCA of the Twin Tiers’ partnership with the Western New York Integrated Care Collaborative. The coordinator leads social care and population health initiatives, including programming related to the New York State Medicaid 1115 Waiver. The role connects community members with appropriate services, supports evidence-based health programs and older adult wellness, and helps advance health equity across the service area.

What you'll do

• Lead YMCA operations and compliance related to the NYS Medicaid 1115 Waiver
• conduct social determinants of health screening and referral intake
• triage referrals and connect community members with community organizations and healthcare providers
• document services in the integrated care platform
• maintain referral partnerships
• support volunteers and grants
• contribute to performance reporting
• and support older adult and priority-population programs.
