What you'll do

• Serve as a telephone liaison for referral sources, agencies, health professionals, and field staff
• receive calls and referrals
• provide basic service information
• coordinate care initiation with patients and families
• complete intake screening and document information for preliminary admission decisions
• prepare reports and enter data
• support field staff scheduling
• where permitted by law, receive verbal physician orders
• maintain patient files and employee files
• coordinate with clinical staff on practice standards and regulations.
