What you'll do

• Coordinate services for an assigned caseload of children and work with caregivers, the child's care team, and community providers.
• Complete needs assessments and individualized plans of care, support treatment adherence and preventive care, coordinate referrals and transitions from hospital to home, advocate for clients, and maintain required records in care management and electronic health record systems.
