What you'll do
- Manage a caseload

- maintain Health Risk Screening Tool information and individual support records
- participate in team meetings and support plan updates
- arrange and attend medical appointments as needed
- monitor outcomes and provide direct service as needed
- advocate for independence
- supervise and mentor Direct Support Professionals and work with Life Sharing Providers
- monitor program sites and files for regulatory compliance
- handle office records, timesheets, expenses, incident reporting, billing units, and required communications.
