What you'll do

• Assess physical, psychosocial, and spiritual needs of patients and families
• provide intermittent skilled nursing under physician direction
• monitor pain and symptoms and help patients and families manage them
• liaise with physicians
• provide emotional support and health teaching
• refer patients and families to services and follow up
• complete hospice plans and reports
• participate in care conferences, bereavement follow-up, case review, and quality improvement
• clinically supervise HNA activities related to patient care
• participate in scheduled on-call rotation
• assist with discharge planning and plan-of-care coordination.
