What you'll do

• Provide skilled direct patient care within RN scope, including listed wound, catheter, infusion, tube, ostomy, and other procedures
• implement the case manager’s plan of care
• document care accurately and on time
• educate patients and caregivers
• coordinate with other disciplines
• report patient information to the case manager and provider
• attend case conferences
• maintain patient privacy
• participate in on-call duties as required by weekend rotation.
