What you'll do

• Assess clients and perform nursing procedures
• review client-to-staff skill matching and visit clients to evaluate care and clinical notes
• develop and evaluate plans of care with patients, representatives, and caregivers
• obtain and implement physician orders
• educate clients and families
• document progress
• coordinate with the interdisciplinary team and physicians
• supervise team members
• coordinate equipment and referrals
• and participate in quality improvement and training activities.
