What you'll do

• Build relationships with hospital case managers, social workers, physicians, and staff
• educate referral sources about home health services, programs, and eligibility
• meet patients before discharge and explain services, insurance, and eligibility
• advocate for safe discharges
• coordinate hospital-to-home care with providers and caregivers
• process referrals and admission documentation
• track referral activity, conversion rates, and barriers
• collaborate with intake, clinical, scheduling, and account executive teams.
