The role supports safe, individualized services through nursing plans, medication oversight, clinical coordination, documentation, and education for individuals and staff.
What you'll do
- Perform nursing duties within the NYSED-licensed Registered Nurse scope of practice
- assess individuals’ health needs and coordinate with healthcare providers, service providers, clinical staff, and program staff
- participate in program planning and create and update Plans of Nursing Service
- document health concerns, treatments, and recommendations and complete follow-up
- interpret medical tests and screening results for staff and team members
- participate in admission and transfer/discharge planning
- review medications and monitor effects
- evaluate and monitor self-administration of medication programs
- coordinate medical and clinical appointments and follow-up
- review medication errors and make recommendations
- provide health and dietary education and staff training
- assist with staff TB testing and monitoring. May provide direct or clinical supervision to assigned LPNs as directed.
