What you'll do

• Manage assigned patient visits and on-call schedule
• respond to patient and caregiver calls and determine appropriate clinical interventions
• make skilled nursing home visits as needed
• assess patient status and changes requiring intervention
• evaluate ongoing home health needs and care plans
• provide skilled nursing care under physician orders and individualized plans
• perform procedures within RN scope
• administer medications and treatments
• communicate changes to physicians and care-team members
• coordinate care
• educate patients and caregivers
• maintain timely clinical documentation
• identify and coordinate additional services, equipment, supplies, or resources
• protect confidentiality and comply with HIPAA and regulations
• travel locally throughout Bergen and Essex Counties during scheduled on-call hours.
