Individual Support Case Manager — Part-Time, Various Locations

What you'll do

• Work with the participant’s care team
• provide resources that support safety and independence in the community
• assist with training and supervision of caregivers and personal care assistants
• conduct home visits and document activity
• complete service-related administrative duties and required documentation
• transport clients as appropriate
• follow individual service plans and service policies.
