What you'll do

• Lead and manage team performance
• support evaluations and care conferences for independent and assisted living residents
• oversee and provide resident care and communicate with families as appropriate
• conduct admission and ongoing evaluations quarterly and after specified changes or returns from care settings
• obtain written orders for care-plan changes
• verify medication and treatment administration records and coordinate with pharmacy
• review occurrence reports
• schedule resident health education
• communicate emergency policies
• support continuous improvement.
