What you'll do

• Evaluate and regularly reassess nursing needs
• initiate and revise plans of care
• provide specialized nursing and preventive or rehabilitative procedures
• prepare clinical notes and care conference summaries
• coordinate services and report changes in patient condition
• counsel patients and families
• process orders and complete certification, recertification, and discharge summaries
• refer patients to other clinicians as needed
• supervise and teach nursing personnel
• complete OASIS assessments
• take on-call duty nights, weekends, and holidays as assigned.
