What you'll do

• Participate in resident care and quality-of-life policy review
• support social work policies and quality assurance meetings under the Director of Social Services
• maintain required social service documentation and document psychosocial progress
• work with the interdisciplinary team to promote resident rights and psychological well-being
• complete social histories and psychosocial assessments
• participate in interdisciplinary care plans
• provide therapeutic interventions and support residents’ adjustment to long-term care
• educate and support residents and families and refer to agencies as appropriate
• coordinate discharge planning and home care referrals under direction.
