The role supports patient independence and functional goals through evidence-based care, collaboration with physicians and the interdisciplinary team, patient and caregiver education, and accurate clinical documentation.
What you'll do
- Perform comprehensive evaluations

- maintain clinical documentation that meets regulatory and reimbursement requirements. Daily travel within the service area to provide care in patients’ homes, assisted living facilities, or nursing homes is required.
