What you'll do
- Provide direct in-person patient care during on-call and after-hours shifts.
- Conduct initial and ongoing assessments of physical, emotional, and psychosocial needs.
- Develop and revise Hospice Plans of Care in collaboration with physicians.
- Educate patients and families on disease progression and symptom management.
- Maintain accurate clinical documentation and support regulatory compliance.
