Home Care Registered Nurse (RN) Case Manager

The Home Care Registered Nurse (RN) Case Manager role is field-based in Burlington, MA, serving clients throughout the Boston Northwest area, including Bedford, Lexington, Burlington, Woburn, and surrounding communities. Pay is $45–$52 per hour. The role supports older adults who want to remain safe and independent at home. The nurse conducts assessments, develops care plans, coordinates follow-up, and supports clients, families, and caregivers. Working with the Director of Nursing and care team, the case manager helps maintain continuity of care and respond to changes in clients’ needs.

What you'll do

• Conduct in-home nursing assessments and reassessments
• develop, review, and update individualized care plans
• provide case management and follow-up visits
• monitor changes in client condition and coordinate clinical follow-up
• educate and support caregivers
• serve as a clinical resource
• collaborate with nursing, scheduling, and care management teams
• communicate with physicians and other professionals when appropriate
• support care transitions
• maintain timely clinical documentation
• build relationships with clients, families, caregivers, and community partners.
