What you'll do

• Work with families to identify care coordination needs and services

• coordinate services in homes, schools, and communities
• ensure care plans are appropriate and meet safety and treatment needs
• build relationships with child and family teams
• develop partnerships with community resources
• maintain documentation
• participate in program evaluation, meetings, and training
• remain current with required training.
